Terms and Conditions:

1. Authorization to Perform Repairs: By signing below, the customer authorizes Uni-Tech Paint & Body Shop to perform the necessary repairs on the vehicle listed above. Any additional repairs not included in the original estimate will be communicated and require additional authorization.

2. Parts and Materials: Replacement parts may be new, used, or aftermarket as necessary. If specific parts are requested, they may be subject to additional costs.

3. Payment: Full payment is required upon completion of the repairs. Accepted payment methods are cash, credit card, or certified check.

4. Storage Fees: If the vehicle is not picked up within 5 business days of repair completion and invoicing, a daily storage fee of $75 will apply. Storage fees will not exceed 30 days, at which point legal actions may be taken in accordance with Georgia state law.

5. Abandoned Vehicle: If the vehicle remains unclaimed for more than 30 days, Uni-Tech Paint & Body Shop reserves the right to take legal action as permitted under Georgia Code Title 40 - Motor Vehicles and Traffic, including filing a mechanic's lien for unpaid repairs and storage fees.

6. Liability: Uni-Tech Paint & Body Shop is not responsible for loss or damage to the vehicle or its contents due to fire, theft, accident, or any other cause beyond the shop's control.

7. Permission to Test Drive: I authorize Uni-Tech Paint & Body Shop to operate my vehicle for the purpose of testing and verifying repairs.

8. Photo Authorization: I authorize Uni-Tech Paint & Body Shop to take photographs of my vehicle for documentation and/or marketing purposes.

9. Authorization for Third-Party Payments: I authorize my insurance company (if applicable) or any third-party payer to issue payments directly to Uni-Tech Paint & Body Shop for the repairs and services performed on my vehicle. I understand that I am responsible for any remaining balance not covered by the third-party, including deductibles and additional charges.

10. Inspection and Delivery: The customer agrees to inspect the vehicle upon pickup and acknowledges that the vehicle has been satisfactorily repaired by signing the release below, including during towing by third-party services.

11. Warranty Information: Beltline Auto provides a lifetime warranty on all repairs performed. This warranty covers defects

    in materials and workmanship for as long as you own the vehicle. The warranty does not cover

    normal wear and tear, misuse, or damage caused by external factors. It also does not cover

    subsequent damage that is unrelated to the original repairs.

12. Personal Property: Beltline Auto is not responsible for any personal items left in the vehicle. Customers are advised

    to remove all valuables and personal belongings before leaving the vehicle for repairs. Any

    items left in the vehicle are left at the owner’s risk.

Disclosure Regarding Electronic Signatures and Records

By signing this form electronically, you agree to the use of electronic records and signatures. You understand and consent to the following:

1. This consent applies to the Repair Authorization Form for Uni-Tech Paint & Body Shop.

2. You may request a paper copy of this document at no charge by contacting us at admin@unitechbodyshop.com.

3. You may withdraw your consent to use electronic records and signatures at any time by notifying us in writing at admin@unitechbodyshop.com. Doing so may delay the repair process as paper forms will need to be used.

4. To access and sign this form electronically, you must have a device with internet access, a web browser, and an email address.

5. By clicking "Submit" or signing electronically, you confirm your consent to the terms above and your agreement to use electronic records and signatures.
